The label schedule, one phase at a time

The Zepbound label starts with 2.5 mg once weekly for four weeks, followed by 5 mg once weekly. Later increases use 2.5 mg steps after at least four weeks on the current dose. Those minimum intervals do not mean every person progresses through every strength. For weight management, the label lists 5, 10, or 15 mg as maintenance doses; for obstructive sleep apnea, it lists 10 or 15 mg. The maximum is 15 mg weekly. Zepbound label

A calendar week and a dose phase answer different questions. The calendar records when something happened; the phase records which prescribed amount applied at that time. Keeping both visible makes a period at the same strength distinguishable from an escalation. A higher row in the reference table is not a notification to change your medication.

Pen versus vial: the amount and the container

The current Zepbound label includes single-dose pens, single-dose vials, multi-dose vials, and single-patient-use KwikPens. Their labeled volumes differ. PinPoint can track vials and prefilled pens side by side and estimate when supply will run out. For a hypothetical separately supplied compounded tirzepatide vial, 10 mg in a final volume of 2 mL equals 5 mg/mL. On a U-100 scale, 20 units is 0.20 mL, so the contained amount is 1 mg. This is arithmetic only, not a preparation instruction or a Zepbound dose. It does not establish that a compounded product is equivalent to the branded medicine. Does the table mean a dose increases every month?

No. The label sets a minimum of four weeks at the current dose before later increases. Maintenance selection depends on response and tolerability, so the table does not assign an automatic escalation date. Zepbound label
